During these consultations we will:

• Provide a medical assessment to determine the appropriate form of transportation
• Review and determine insurance benefits
• Develop a patient transport plan specifically tailored for each individual

Some of the information that we require from you when booking a flight are:

• History and Physical
• Face Sheet/Patient Demographics
• Insurance Information (to determine benefits for the patient)
• Medication Administration Records (MARS)
